By clicking the “I accept” button below, I HEREBY REPRESENT THAT I HAVE READ AND UNDERSTOOD THIS AGREEMENT AND AGREE TO BE BOUND BY ITS TERMS
Billing for the quarterly fee will be automatic and you must sign up for the subscription before you can begin to use the service. You can cancel at any time. If you are in the middle of a quarter, your subscription is valid through the end of the quarter.
Rates are per the table below. Booking fees are $8 per occurred booking.

Invoices are available under “My Account” on the TDS website.
Invoice balance is paid automatically when your membership renews every three months. If a subscription is cancelled, booking fees will be automatically charged at the end of the quarter.
You cannot schedule a TDS dental professional without using the scheduling system.
TO THE EXTENT PERMISSIBLE UNDER APPLICABLE LAW, WE MAKE NO WARRANTIES IN CONNECTION WITH OUR SERVICES OR THE SERVICES PROVIDED BY SITTERS AND HEREBY DISCLAIM ALL WARRANTIES OF ANY KIND, WHETHER EXPRESS, IMPLIED OR STATUTORY. You agree to indemnify and hold TDS harmless from and against any and all claims by you or any third parties arising out of or relating to the acts or omissions of any dental professional. In no event will TDS be liable to you for any punitive, special, incidental or consequential damages in connection with this Agreement or the acts or omissions of any Dental professional for any reason whatsoever.
Cancellation Policy:
In the event of a patient cancellation / early dismissal, you are still expected to pay the dental professional for the time scheduled.
You cannot, under any circumstances, share a dental professional’s contact information with any individual outside of Tempus Dental community.
TDS expects that Dental office will treat Dental professionals with courtesy and civility. Rude or disrespectful treatment toward the Dental professionals will not be tolerated. TDS will address intolerable behavior with the Dental office once. Regardless, Dental professionals are free to discontinue their services at any time with your Dental office and you are free to discontinue service with any Dental professional in the TDS network at any time. If a dental office divergent behavior continues, TDS will inform the Dental office by phone or email.
Being respectful of dental professionals time and schedule is expected. Please respect their schedules like you would your own.
If TDS makes any changes to this agreement, the Dental office will be asked to read and agree to the changes.
